What is a MU Form?
The MU forms are uniform licensing forms used by all the states in connection with licensing mortgage companies and loan originators. The MU1 Form is to provide information about the company in general, such as company name and DBAs, contact information, ownership, etc. The company will only have one MU1 Form. If the company information changes, they must provide a new MU1 Form. The MU2 Form is to provide information about the people who own or control the company and branches. Each person will have his or her own MU2 Form, so while a company only has one MU1 Form, it may have many MU2 Forms. The MU3 Form is for information about the branches that that company operates. A company will need to submit a MU3 Form for each branch where it wants to make Oregon residential mortgage transactions.
