A Missing Record Inquiry is a form in Nursys® that a nurse will complete to resolve missing license information issues. To access this form, click the Send Missing Record Inquiry link. Provide all necessary information including the board of nursing that the form should be sent to then click Send. The information will be forwarded to Nursys® from your licensing board of nursing as they submit their licensure updates to Nursys®.
