What is a mishap investigation board?
Aviation: Squadrons have a standing Aviation Mishap Board (AMB) that trains as a unit. The AMB usually consists of the executive officer (senior member), the safety officer, an officer well qualified in aircraft operations, and officer well qualified in aircraft maintenance, and a flight surgeon. In the event a squadron has a Class-A mishap, these people usually are appointed to the mishap board. A naval aviator or naval flight officer, in the grade of commander or lieutenant colonel, usually replaces the executive officer from outside of the chain-of-command that is senior to the senior person involved in the mishap. The flight surgeon usually is assigned from the wing. Appointments are under the authority of the controlling custodians who usually are two or three-star admirals and generals.
