A Health Risk Assessment (HRA) is a computerized questionnaire that asks questions about your health, lifestyle behaviors and a section on biometric testing such as height, weight, body composition, blood pressure, resting heart rate, cholesterol, glucose and flexibility. The HRA utilizes all this information to provide you with a Personal Wellness Profile (PWP). When you complete a HRA, you will receive a $20.00 credit on your Wellness Advantage Debit card.
