What is a Federal Authorization Form (Title IV)?
If you have Federal Financial Aid such as Federal Direct Loans, Perkins Loan, Pell Grant, SEOG, Federal Academic Competitiveness Grant, and the Federal Smart Grant; the College is required to obtain your authorization to pay non-institutional charges. Examples of non-institutional charges would be Flex Dollars, Computer Lab Paper Usage Fees, Dorm Damage, Returned Check Fee, Flight Fees and Health Insurance.
