If you have Federal Financial Aid such as Federal Direct Loans, Perkins Loan, Pell Grant, SEOG, Federal Academic Competitiveness Grant, and the Federal Smart Grant; the College is required to obtain your authorization to pay non-institutional charges. Examples of non-institutional charges would be Flex Dollars, Computer Lab Paper Usage Fees, Dorm Damage, Returned Check Fee, Flight Fees and Health Insurance. Please click here to print the Title IV Authorization Form and return the completed form to the Financial Aid Office.
