What is a docketing statement and where can I get one?
A docket statement is an informational form you must complete and file with the court of appeals shortly after giving written notice of appeal. The information contained in the docketing statement is used by the clerk to set up the appropriate computer files used in tracking your appeal. Both civil and criminal docketing statements may be downloaded from the court’s web site.
