A DMA-6 is the Department of Medical Assistance form on which the physician verifies an individual’s need for Personal Care Home care. All Personal Care Homes that receive Medicaid funds must forward the completed DMA-6, signed by the prospective resident’s physician, to the Georgia Medical Care Foundation for review and approval before Medicaid payments can be made. The physician completing the DMA-6 must be licensed in Georgia.
