A CONTACT LENS EVALUATION DETERMINES IF THE FIT AND PRESCRIPTION OF THE CONTACT LENSES IS APPROPRIATE. A CONTACT LENS PRESCRIPTION IS ONLY VALID FOR ONE YEAR, IN NEW YORK STATE. OUR OFFICE FEE IS $40.00 AND IS COLLECTED IN ADDITION TO THE FEE FOR A COMPREHENSIVE EYE EXAMINATION.
