What is a CCM Care Plan?
A Care Plan is a comprehensive written document, prepared by the parent(s) and care manager, that specifies the personal preferences, activities, health status, work, and developmental achievements to be maintained for the disabled individual. As a first step and with the assistance of a professional, parents will prepare this plan. This document is crucial in planning for the child’s future, yet we know that many emotional and family considerations make it difficult to prepare alone. It will assist the social worker in decision making and coordination for the special needs individual.
