When the claimant’s condition is medically stationary, the carrier shall close the claim by issuing a “Notice of Closure: Own Motion Claim” (Form 440-2066) to the claimant, claimant’s attorney, if any, and the Workers’ Compensation Division. OAR 438-012-0055.
