What insurance endorsement forms are preferred and/or acceptable to CLC?

CLC prefers the Grantor of License endorsement form (ISO form CG 20 36 10 01), but will also accept the Designated Person or Organization endorsement (ISO form CG 20 26 1185 or CG 20 26 0704). The endorsement form must include the policy number, be signed by the agent and attached to the insurance certificate. CLC will only accept a Grantor of License or Designated Person or Organization endorsement (either ISO forms or equivalent if the carrier does not use ISO forms). CLC WILL NOT accept a Vendors endorsement, Grantor of Franchise endorsement, or Owners, Lessees, or Contractors endorsement. The contractual relationship between CLC and the licensee does not fall within the parameters defined by these type endorsements.
