What insurance does Lake Orthopedic Group accept?
Lake Orthopedic Group will file all insurance claims as a service to our patients, providing you bring the insurance card with the claim address, phone number, group number, and ID#. If your insurance requires a referral from your PCP (Primary Care Physician), it is your responsibility to obtain this and bring it with you to your appointment. You will also need to let our staff know if your insurance changes.
