The funeral director will need the following information regarding the deceased : • Name, address, birth date, and birthplace • Personal Health Number • Social Insurance Number • Spouse’s full name (maiden name in the case of a wife) • Most recent occupation • Father’s name and birthplace • Mother’s maiden name and birthplace • Name and address of executor, legal representative, and next-of-kin
