The information that you give us on the application form, plus details of the difficulties experienced because of the disability. For those children who see a paediatrician, there will be a number (given to us by Health) that matches the International Coding of Diseases (ICD) description of the condition/s the child has. (Conditions such as Autism, Downs syndrome and Epilepsy etc.
