WHAT INFORMATION SHOULD BE REPORTED ABOUT CHILD ABUSE AND NEGLECT?
* Report information about the child’s identity if known, i.e., name, address, age, gender, parent’s names and addresses, other children in the home, and phone numbers, name of the alleged maltreater, along with address, phone, gender, age, work place. Do not worry if you do not know all of the information or even the child’s name. We may have other information about the child or incident and be able to identify the child. * Describe the maltreatment, surrounding circumstances, and frequency. Describe why you think that the child was or is being abused. * Describe the kind of injuries, i.e., bruises, cuts, broken bones, etc. and where on the child’s body the injuries occurred. Describe who or what caused the injuries. Was an instrument such as a board, a belt, a paddle, or other object used to hurt the child? Was in infant or small child shaken? * Describe the conditions in the home that make it unsafe or unhealthy. * Describe how long a child was left unsupervised. * Describe what you
