Eligible applicants should first submit a completed Department of Defense Form 2944, Claim for Retroactive Stop Loss Payment, available at the “Claims Process” link of this Web site. AFPC or ARPC personnel officials will review the application and attempt to adjudicate the claim based on the information provided. Applicants will be notified should additional supporting documents be needed.
