All other petitioners must provide: • Identifying and contact information; and • A proposed class definition; and • A description of why the proposed class should be added to the SEC (e.g., records and information available are inadequate to estimate the radiation doses acquired by members of the proposed class of employees with sufficient accuracy; identification of any exposure incident that was unmonitored, unrecorded, or inadequately monitored or recorded if the petition is based on an exposure incident, etc.). Please Note: Petitioners may be required to provide evidence that the incident occurred if NIOSH is unable to obtain records or confirmation of the incident form sources independent of the petitioners. This evidence is not required at the time the petition is submitted
