What information must be on a Sales Disclosure Form?
A Sales Disclosure Form must have the following information: 1) Sellers name and address 2) Buyers name and address 3) Address of property being conveyed (sold) 4) Date property was sold 5) Sales price of property and parcel/key number 6) Check conditions of real estate that apply to sale (if any) 7) Signatures, phone numbers of buyer and seller, date signed 8) Preparer’s name, address, and phone number
