• Name of Insured – must be listed exactly as listed on the Division of Corporation’s Certification • Policy Number – binders are not acceptable • Coverage Limits – (see amounts listed above) • Workers’ Compensation Coverage • Effective and Expiration Date of Policy • Certificate Holder – DBPR/Electrical Board
