What information is required when making a referral to the ISA?
The referral form outlined the information and documents you should provide at the time you make a referral. If you do not hold the information requested on the form then this particular section of the form should be left blank. Employers are not required to obtain information from other sources or draft any documents to meet it’s responsibilities to the ISA. If you are aware of relevant information that is held by another source you should indicate this where required on the ISA referral form.
