You will be required to complete a 2 page AIA Individual Hospitalization Claim Form. The first page will be the Insured’s details and declaration and the second page must be filled by your treating doctor. A copy of this form can be obtained by calling the AIA Medical Assistance hotline, or from your agent or any of AIA branches.
