What information is protected under the Privacy regulations?
“Protected health information” under the Privacy regulations is personally identifiable health information, in any form (verbal, written, or electronic) that is used or disclosed by a covered entity. This includes, but is not necessarily limited to, names; specific dates (e.g. birth, admission, discharge, death); telephone numbers; Social Security numbers; Medicaid numbers; medical record numbers; and photographs.
