What information is collected if I sign a release of medical information form?
• There may be many medical release forms for you to sign throughout the course of the study. These forms usually give us permission to request medical records for a certain medical test or health problem you told us about. These release forms are usually good for a few months, which are why you may be asked to sign a few different ones, as they will expire. Your Study Site staff should explain what medical facts or records they are sending for before you are asked to sign the release form. Read each form carefully and feel free to ask your Study Site staff questions.
