There are three types of information that will be shared in this process: Hospital Discharge Summaries, Laboratory Test results and Medication History. Below, please find a brief description of each category: Hospital Discharge Summaries: Clinical notes written by the discharging physician at the time of your release from the hospital. These notes outline the course of treatment, your condition at release, and the post discharge expectations and instructions. Laboratory Test results: Your recent laboratory test results Medication History: Your prescription history.
