• First Name, Middle Initial (if any), and Last Name • E-mail address • Office telephone number • Office fax number • Department name • Highest Degree (i.e. BS, MS, Ph.D., etc.) • Year that the degree was obtained You may call OSRA at (305) 348-2494 to provide the information or e-mail us at osra@fiu.edu.
