CT, MRI, MRA and PET scans related to an emergency room visit, surgery (either at a hospital or freestanding surgery center), hospital stay or outpatient observations do not require prior plan approval. Prior plan approval also is not required for “low-tech” procedures such as X-rays and mammograms.
