Three sticker labels have been sent from CTB/McGraw-Hill: 1) two blank labels 2) a barcode label. If the barcode label is placed incorrectly on the booklet, place one of the white labels over the barcode label and then contact your County Test Coordinator for a replacement label. If the barcode label is missing or destroyed, contact your County Test Coordinator who will contact the Office of Assessment and Accountability. The Office of Assessment and Accountability will supply a duplicate label.
