What if the summons is wrong or the person summoned had died?
If the address is incorrect, please note your correct address and return the questionnaire to the Jury Coordinator. We sincerely apologize if you have received a summons addressed to a family member who has died. Jury lists are compiled every four years from Department of Motor Vehicle and Voters Registration records, so often information is not correct. Please return the questionnaire with the appropriate information.
