• Request written consent from the employee to apply any remaining vacation and/or personal credits; • Complete an Employee Transaction Form (ETF) and indicate that a Record of Employment (ROE) be issued to the employee. Then forward the ETF to HR-Records/Payroll and a copy to the EWO. The ROE will assist the employee in applying for Employment Insurance Benefits once they have exhausted all sick credits.
