Please use the HearUSA Hearing Care Network econtact form or call 1-800-999-1458. We’ll be happy to contact the audiologist to see if they would like to participate. They would need to apply and go through our credentialing process prior to being listed in our current provider directory. (Provider directory search results subject to change without notice.
