A. The answer to this question really depends on your qualifications and/or your role. If your sole role is to collect data, you should refer respondents to their clinician/physician. However, if you are acting as the respondent’s health care provider, you could explain that some of the most helpful information is found by looking at each of the items individually to see which ones were scored as being difficult. Often these item scores are useful in goal setting. Over time the patient’s score may change, reflecting a change in the responses to some of the items and these could be examined to see which items have shown improvement.
