What if my doctor prescribes a medication that isn listed on the attached, CHA Health Abbreviated Prescription Drug List?
The CHA Health Abbreviated Prescription Drug List is NOT a complete list of all medications for which CHA Health provides coverage. It is a listing of the most common medications and the categories of those prescription medications. A complete listing of generic and preferred medications is available, the Preferred Drug List. You can request a copy of this list by calling Member Services. If your physician prescribes a medication that isn’t listed on the Preferred Drug List, it is a non-preferred medication or it is not on the CHA Health formulary. Consult your CHA Health Abbreviated Prescription Drug List for a listing of non-preferred medications. If your physician prescribes a medication that is non-formulary, which means that it is not on either list, it requires Prior Plan Approval and, if approved, is covered by the highest copay. Please refer to your Certificate of Coverage for exclusions and your schedule of benefits.
