WHAT IF I’M MATCHING ID INFORMATION TO THE PATIENT’S ID BAND AND FIND IT DOESN’T MATCH?
STOP! Research to know why information isn’t matching. If the patient confirms that the ID band name or birthdate is incorrect or the account/medical record number is incorrect YOU MUST take responsibility to notify nursing staff to make the change immediately and place the correct band on the patient. This is a High Risk patient safety situation requiring action.
