Health care professionals and facilities that participate in Aetna plans (in-network providers and facilities) should file claims on your behalf. Some out-of-network providers may also submit claims on your behalf. Check with your doctor or other health care provider to determine if you need to submit a claim. If you receive a bill, or if you receive care from an out-of-network provider for covered services and need to submit a claim, please complete and mail the applicable form to the address on your ID card. Claim submission forms are available on the Aetna Navigator website or under the Important Materials section on the home page.
