Because the Questionnaire is available from any computer with an internet connection, if you do not complete the Health Risk Questionnaire by 15 September 2010 midnight Central Time, you will pay higher contributions for Motorola Health Plans (Health Investment Plan, Health Advantage Plan, and Health Plan Plus) coverage in 2011. If you did not submit the HRQ before the 15 September deadline, there will be a second HRQ period from 22 September through 5 December 2010 and then again 1 January through 21 January 2011 until midnight Central Time. If the HRQ is completed during these times, you will be eligible for discounted monthly contribution rates starting with your first February 2011 paycheck deduction. You will miss the discounted rate for one month only (January). The site will be closed from 6 December through 31 December 2010 for maintenance in preparation for Separation.
