What if I lose my Premium TransitChek MetroCard?
TransitCenter will replace lost or stolen Premium Cards with a new Card effective the first day of the month following receipt of a signed Certification form, completely filled out by the benefit recipient. No reimbursement is made for the period of time that an Enrollee is without a Premium Card. To obtain a Certification form, send a request to enrollment@transitcenter.com with your mailing address.
