Students who have a physical disability, medical condition or specific dietary need that they believe could affect their room assignment must provide details on the Residence Information Form and medical documentation. The medical documentation must be received in the Department of Housing and Conference Services by the June 17th, 2009. The medical documentation can be mailed to Department of Housing and Conference Services, 1125 Colonel By Drive, Ottawa, Ontario, K1S 5B6 or faxed to 613-520-3952. While this information will be considered in the room assignment process, not all special consideration circumstances warrant a single room or suite style room assignment. Students may also be requested to provide further documentation or have a consult with staff at Health and Counseling Services.
