If you earn a higher reimbursement level of Wellness Credits during December, the increase in the contribution to your Marathon Health Reimbursement Account will be added by February 1, 2008. What if I need a card for my spouse? Employees can request additional cards for spouses or dependents; the first additional card is at no cost, however, each additional card thereafter has a $5 fee, incurred by the employee. Additionally, if you request a new card to replace a lost or stolen card, you will incur a fee of $12 per card. If you have a Marathon Health Reimbursement Account, you will receive a notice in late December from Marathon indicating your beginning balance.
