What if I don’t know the answers to certain questions on the questionnaire or can’t locate all of my medical records?
Simply do the best that you can. Only questions to which blanks were filled in or where answers were given in the affirmative will show up to the end user (i.e., healthcare provider). Questions to which no response or a negative response is given don’t typically show up to the end user. This is a more efficient way to present your story. You can always go back and change or add information to the questionnaire when new information is acquired from the doctors, hospitals, labs and imaging facilities. There will be plenty of information for you to enter in the beginning, and this will be an immense help to those that care for you. You can get copies of your medical records from your doctor. Contact your doctors and hospitals to see how you can get copies of the records.
