What if I can schedule an appointment for a physical before the July 15 deadline to turn in the medical history form?
If you are unable to set up a date for a physical before the deadline please copy the pages you have filled out and mail them to Saint John’s Health Center. Attach a note with the date you are scheduled to have your physical. Take the original form with you to the appointment for your physical. Your physician will perform the physical exam, review all of your information, and sign in the appropriate areas. Send your form to St. John’s Health Center as soon as it is completed.
