[Return to Top] If your course requires proctored exams, and you cannot come to one of the campus locations to take your exam, you will need to make your instructor aware of this situation and secure a qualified proctor with the instructor’s permission. Click here to view Proctor qualifications. A test proctor form must be completed and mailed to Virtual Campus for approval three weeks prior to your requested test date. The form will be forwarded to your instructor for approval. Click here to view a printable copy of the Test Proctoring Form. (You will need Adobe Acrobat ReaderĀ® to view these files.) Print out the form and follow the instructions on the form. If you would like a form mailed to you, contact the Virtual Campus Office.
