Most carriers require certain documentation for proof of business. The documentation generally is very similar between carriers and is located on the reverse side of the Broker Group Census Forms. In addition to documentation required by the carriers, SBSB also requires documentation for membership submission. If an employer cannot provide the necessary documentation for proof of business, you should contact SB TeleBrokerage for assistance. If the documentation submitted to SBSB clearly does not meet the requirements of the plan, SBSB may not be able to submit the case. In some questionable cases SBSB will confer with the plan directly for approval or denial of the case.
