In such cases, the student should preferably submit a DD Form 295, Application for the Evaluation of Learning Experiences during Military Service, a DD Form 214, Certificate of Release or Discharge from Active Duty, or course completion certificates. The DD Form 295 is available to all active duty servicemembers and may be obtained from Education Centers at the onsite duty location.
