The best way to ensure that anyone receives the type of care they want is to discuss their choices with their Substitute Decision Maker and family. This is because these people will be the ones called by the doctors when someone becomes unwell. Also, all parties should have a copy of the Advance Care Plan so this can be brought to the attention of the treating doctors.
