When clients need to report a change to their WHP case, providers can assist clients with filling out and submitting Form H1019 – Report of Change: • Form H1019 – Report of Change • Form H1019-S – Report of Change in Spanish Form H1019 – Report of Change Instructions Case name – Enter the head of household name as it appears on the WHP Medicaid Identification card. Case number(s) – Enter the case number of the WHP client. Advisor name – Enter the name of the HHSC eligibility worker processing the case.
