What ID cards are provided for FSBP members?

The Plan will issue you a combined FSBP / Medco Prescription ID Card with the PPO Network name and logo on it. When you visit your providers’ offices in the United States, you must show them your Health Plan ID card prior to treatment confirming PPO participation in order for you to receive PPO benefits. Your Member ID is a randomly selected number. The Plan does not use Social Security Numbers for privacy reasons. The card also has Medco prescription information on it indicating you are covered under our Prescription Program. You must use it at participating network pharmacies to obtain your prescriptions whenever you are in the U.S. Carry your FSBP / RX ID Card with you at all times.
