If you are hired on or after August 10, 2009 you do not need to complete the Questionnaire. You’ll automatically pay the discounted contributions for a Motorola Health Plan. However, the HRQ will be accessible beginning August 24 and will remain available through January 18, 2010 if you are interested in assessing your current health status, enrolling into a health coaching program or completing any Health Education Centers.
