If you are hired on or after 7 August 2010, you do not need to complete the Health Risk Questionnaire. You’ll automatically pay the discounted contributions for a Motorola Health Plan. However, the HRQ will be accessible beginning 23 August through 15 September and again from 22 September through 5 December 2010 and again 1 January 2011 through 21 January 2011 if you are interested in assessing your current health status, enrolling in a health coaching program or completing any Health Education Centers.
