If you fail the Completion Form of the PSA (Activity), you will be given one more chance to complete the form successfully. You will be given feedback on your form by the PSA team regarding areas in which you can improve upon. If you fail the Completion Form stage for a second time, then you will unable to complete the PSA (Activity) as a whole.
