What happens if i do not obtain preauthorization?
• Whenever Preservice Review has not been performed you will be required to pay an additional amount which will be lesser of $250 or 50% of the allowed amount. This additional amount of $250 or 50% of the allowed amount is in addition to any other payment requirements of this Policy and does not apply toward satisfying your calendar year Deductible or out-of-pocket maximum.
